What to Expect on Your First Visit
  Be sure to bring your insurance cards and at least one form of picture identification (Driver's license or Passport).
  Please arrive 15-20 minutes prior to your appointment time.
  Your visit time from the moment you sign in to the time that you leave can range from 1 to 2 hours in duration, possibly longer if an X-ray or MRI is to be performed.
  If a recent X-ray or MRI was performed on the injured area, please bring them with you. (A new X-ray or MRI may be needed on your first visit)
  There is a fee of $4.00 for parking.

    Patient Consent Form for HIPAA (after reading Notice of Privacy Practices)

    Update History (prior patients with new problem)
